I understand that I am agreeing to coaching, wellness, and fitness services provided by Sustainability Performance and Physical Therapy, inc. I understand that my program is designed to address my strength, mobility, flexibility, and aerobic capacity, and that it is NOT physiotherapy/physical therapy or any other medical service designed to treat any pain or other dysfunction of the musculoskeletal system.

I am aware of the risks of taking part in the provided program, and I further agree that Sustainability Performance and Physical Therapy, inc,  shall not be liable or responsible for any injuries to me resulting from my participation in the fitness and strength program (whether at

home, outdoors or in any fitness facility), and I expressly release and discharge Sustainability Performance and Physical Therapy, inc and its' employees from all claims, actions, judgments and the like which I or my he

Photographs may be taken for educational tools and social media. By signing below I consent to the use of these photographs in a professional manner.

I HAVE READ THE ABOVE CONSENT, WAIVER AND RELEASE, I AM AGREEING TO THIS CONSENT, WAIVER AND RELEASE OF LIABILITY. IT IS MY INTENTION TO EXEMPT AND RELIEVE SUSTAINABILITY PERFORMANCE AND PHYSICAL THERAPY, INC  FROM LIABILITY FOR PERSONAL INJURY, PROPERTY DAMAGE OR WRONGFUL DEATH BY ANY CAUSE.
